Dental Premier Plan includes:
- Bitewing X-rays (1 per year)
- Regular Cleaning (2 per year), No periodontal therapy
- Dental exams (2 per year)
- 10% off all other dental procedures
Provisions of the Plan
- Fees are guaranteed for one year.
- Initial fee is nonrefundable.
- Payment is due at time of services.
- Patient's responsibility to make and keep their own appointments.
- $25 fee for broken appointment without 24 hour notice.
- Our plan is for you and is not transferrable to another party.
This is not dental insurance. Enrolling in our "In-House" dental saving program gives you the opportunity to obtain your dental treatment at reduced prices. Cannot be used with any other insurance.
